How To Access Accommodations at Grenfell Campus
New students with disabilities, or those with short-term illnesses and injuries, who require an accommodation at the Grenfell Campus should arrange to meet with ACES staff before you start classes for an intake interview. Students who require accommodations or services once they are attending Grenfell Campus should contact the Centre as soon as possible to arrange an appointment.
Once the student has completed the intake interview, he/she will be provided an INFORMATION RELEASE FORM (Blue Form). Following the intake interview, the student must meet with Brad Elliott, Student Affairs Officer with ACES. The student will receive an orientation on the accommodations process at Grenfell Campus. They will also be provided with two forms, the Test/In-class Assignment Form (Yellow Form) and Learning Centre Registration Form (Green Form). See below for more information on these forms.
Students already registered at the Centre should identify themselves to ACES staff at the beginning of every semester, since Blue and Yellow forms have to be completed for each semester.
INFORMATION RELEASE FORM (Blue Form)
This form serves as the official documentation confirming that the student has a documented disability and is approved to receive accommodations. A copy of this form must be processed and forwarded to the student for each term, and a copy of this form must be presented to professors for their records.
All requests for academic (testing) accommodation must be based on documented need. It is the responsibility of the individual requesting an academic accommodation to provide the necessary documentation to Student Services.
This form outlines the accommodations that are recommended for the student that semester. This form will be prepared for each course requiring accommodations. ACES staff will consult with students as to the most appropriate accommodations in their specific cases (i.e., the accommodations requested in particular courses could vary depending on the method of evaluation used by course instructor(s).
Once the student signs the form(s), they are asked to deliver them to their professors. Students are strongly encouraged to meet with their professors, lab personnel, etc., at the beginning of the semester to discuss the arrangements outlined on the form.
Test/In-class Assignment Request Form (Yellow Form)
The Test/In-class Assignment Request Form is provided by the Student Affairs Officer, once the student has received his/her Blue form. The Student Affair Officer will prepare a form for each course the student is requesting an accommodation in that semester. The student is required to fill out Section 1 of the Yellow Form, requesting the student's name, student number, and course information.
Once the student completes Section 1 of the Yellow form(s), he/she is asked to deliver them to his/her professors along with the Blue form. Each professor is required to fill out Section 2 of this form, which provides exam information for each unit test/midterm/in-class assignment the student will write in the course (e.g. length of exams, material allowed, etc.) The completed form must be returned to the Student Affairs Officer for his records.

Learning Centre Registration Form (Green Form)
This form is required to be completed by every student requesting accommodations at Grenfell Campus.
- The student will provide his/her course information on this form and accommodations needed for each course*, along with all of their scheduled exam dates.** The Student Affairs Officer will also require a printed copy of the student’s timetable.
- The student will provide his/her contact information, in order for the Student Affairs Officer to make contact regarding exam details.
In order for the Student Affairs Officer to schedule any exams for a student requiring exam accommodations, a copy of the student’s Blue, Yellow, and Green forms is required.
* It is the student's responsibility to notify the Student Affairs Officer of any courses that he/she drops throughout the semester.
** Students are required to provide their exam dates to the Student Affairs Officer for scheduling purposes. If the student does not receive this information from their professor at the beginning of the semester, they must inform the Student Affairs Officer of the dates as soon as they are provided.
